Disease Management Strategies

St. John’s Medical Management is dedicated to helping people live healthier lives.  If your employees have been diagnosed with a chronic disease or are pregnant, the St. John’s Disease Management Program can help them lead healthier lives and avoid costly trips to the emergency room or hospital.  Highly skilled nurses and clinicians reach out to help educate and motivate individuals to self-manage. Individual counseling, assessment and group classes (in certain geographical areas) are also available with these programs. 

St. John’s Medical Management has partnered with your company to offer individuals education, guidance, and tools to help them manage disease and keep health on track.  Encourage employees who have one of the conditions listed below to, call 800-872-3495 (the number on their ID card) to speak to a Disease Management registered nurse. 

St. John’s Disease Management Programs include

  • Arthritis
  • Asthma
  • Diabetes
  • Congestive Heart Failure
  • Chronic Obstructive Pulmonary Disease (COPD)
  • Depression
  • Maternity

    Participants in St. John's Disease Management programs receive

  • Educational information specific to their condition
  • Follow-up with a registered nurse who assists with understanding medications, diet, exercise and monitoring
  • 24 hour access to a registered nurse
  • Access to classes specific to their disease or condition
